Services are not considered as work comp unless you have completed paperwork with your employer and been assigned a claim number. You will need to give us the correct information in order to bill for services and obtain any authorizations. (See the Worker’s Comp form for required information). If you do not give us this information, you will be held responsible for payment. This is your responsibility. Once we have the information, we will obtain any authorizations. If authorization is not on file, you will be given the option to continue care at your own expense potentially or postpone treatment. If you have enlisted the services of an attorney to represent you, you will need to give us this information as well. If your attorney has filed a case before the Illinois Workers’ Compensation Commission (IWCC), we have an obligation to stop all collection proceedings until your case has been settled or dismissed. At that point in time, it is your responsibility to pay your balance within 15 days of receipt of settlement or contact our Patient Accounts.
